Funding
Full or partial funding for the Firefly may be obtained from medical insurance, the Veterans Administration, non-profit organizations, or through grants. The Firefly is reimbursable under Medicare code E0984 (Firefly power handcycle), and the Dragonfly manual handcycle under code K0108.
If you are unable to use your manual wheelchair pushrims or experiencing shoulder, elbow, wrist, or hand pain, you may be qualified for insurance reimbursement. Please see our “Your Health” page for medical justifications and supporting journal articles.
Our reimbursement specialist works with Medicare and private insurance companies to help get your device at little to no cost to you. Most private insurance companies require pre-authorization and we can assist with that as well.
- Give our reimbursement specialist a call and to verify insurance eligibility.
- If eligible, you will need to be evaluated by a specialist, such as a physical therapist, occupational therapist, or doctor that demonstrates your need for the device.
- Once you have been evaluated, you will need to submit a prescription, medical records, and a Letter of Medical Necessity (LMN)/Certificate of Medical Necessity (CMN). Please see LMN/CMN form and sample.
- Broadened Horizons does not deal your insurance on your behalf. When all the required insurance documents are submitted and pre-approved, your insurance will either send us an order or you can place the order with us and submit it to your insurance for reimbursement.
